ACCEPTABLE BUSINESS IDENTIFICATION

Two forms of acceptable business identification must be submitted. Please include one form of COMPANY and one form of BUYER identification. Please note that acceptable BUYER identification must be submitted for each registrant.

Acceptable forms for COMPANY identification include copies of:

· Retail Sales Tax/Vendor Permit
· Business License for industry related business
· Several recent invoices from industry manufacturers for goods purchased at wholesale
· Store/Commercial lease indicating type of business
· Business advertisement

Acceptable forms for BUYER identification include copies of:

· Imprinted, personalized business card in the applicants name
· Employee I.D. card
· Company credit card with employee and company name identified
· Cancelled company payroll cheque or a copy of a payroll print-out
